At Dowry of Discipleship, we are committed to protecting the privacy of our Supervisees. This privacy policy outlines our practices for collecting, using, maintaining, and disclosing personal information that we receive from Supervisees.
Collection of Personal Information
In order to provide you with the best possible service, when you register as a Supervisee with Dowry of Discipleship, we collect personal information. Only staff who need this information will have access to it. We collect your personal data in a variety of ways, including:
During initial consultations and subsequent Supervision sessions
Through intake forms and questionnaires
Via email, phone calls, or other forms of communication including online forms on our website
The types of personal information we collect may include:
Name, address, and contact information
Date of birth
Work-in-role information
Applicable medical information, including physical and mental health
Information about personal relationships and social circumstances which relate to work-in-role
Any other information that may be relevant to the provision of Supervision services
Use of Personal Information
We will only collect information that is necessary for the direct work of Professional Supervision. Personal information collected from Supervisees for the purposes of providing Supervision services may include:
Understanding working roles and conditions
Helping Supervisees to develop action plans for work-related situations and experiences
Maintaining Supervisee records and case notes
Communicating with Supervisees and other healthcare providers (when necessary)
Billing and payment processing
Disclosure of Personal Information
We do not disclose personal information to third parties except as necessary for the provision of Supervision services, such as:
Disclosing information to work organisations for billing purposes
Third parties who work with Dowry of Discipleship for business purposes, such as accreditation agencies or information technology providers – these third parties are required to comply with Australian Privacy Principles and this policy
Disclosing information as required by a statutory law, such as in response to a court order or subpoena
For the purpose of mandatory reporting according to State Legislation when it is necessary to lessen or prevent a serious threat to a Supervisee or client’s life, health or safety or public health or safety, or it is impractical to obtain the Supervisee’s consent
For the purpose of a confidential dispute resolution process
Dowry of Discipleship will not use your personal information for marketing any of our goods or services directly to you without your express consent. If you do consent, you may opt out of direct marketing at any time by notifying our practice in writing.
De-identified information may be disclosed in Supervision of Supervision, and at such times all efforts will be made to limit shared information so that identification is not possible.
Security of Personal Information
We take reasonable steps to protect the personal information we collect from Supervisees from unauthorized access, use, or disclosure. We maintain physical, electronic, and procedural safeguards to protect Supervisee information. Supervisee case notes are stored using a secure client management portal. Any access to this information is restricted to the staff of Dowry of Discipleship.
Access to Personal Information
Supervisees have the right to access their personal information as necessary. Supervisees may also request, in written form, that their personal information be corrected or amended for our records, subject to legal and ethical requirements. All appropriate requests will be attended to within a reasonable time.
How can you lodge a privacy-related complaint, and how will the complaint be handled at our practice?
We take complaints and concerns regarding privacy seriously. You should express any privacy concerns you may have in writing. We will then attempt to resolve it in accordance with our resolution procedure.
